Vital Records (Birth, Marriage, Death)

The Town Clerk's Office records and maintains all birth, death and marriage records for the residents of Lebanon when the event occurs within the State of Maine.

All Certified Copies can be purchased for $15.00 for first copy, $6.00 for each additional

To request a certified copy of a vital record you must submit a request in writing as well as payment and proof of identity as described on the lower portion of the application form to the Town Clerk's Office in person or by mail to:
